We in the Office for Academic Support view ourselves as a resource to students as well as to faculty and staff. Please review the below resources to learn how we can support you in your work.

CARE Report Form

CARE (Concerning and/or Risky Event) is a system utilized by the Dean of Students & Dean of the College offices to coordinate outreach to students who are identified as socially/emotionally or academically at-risk.

Faculty are encouraged to complete a CARE form when they have concerns regarding a student’s behavior, wellness, class attendance, or academic performance.

Please note that this form should NOT be used in the case of an emergency. For matters of campus safety and security, faculty should contact University Police at 1-508-793-7575.
