Please review this carefully.<\/p>\n\n\n\n<p>UMass Memorial Health Care is an organized health care arrangement (OHCA). This joint notice applies to member organizations and physicians as listed in this brochure.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Patient privacy<\/h3>\n\n\n\n<p>At UMass Memorial, your privacy is a priority. You may also file a complaint with the secretary of health and human services in Washington, DC.<\/p>\n<\/div>\n<\/div>\n<\/div>\n\n\n\n<h3 class=\"wp-block-heading\">Use and disclosure of PHI<\/h3>\n\n\n\n<p>When you visit a UMass Memorial facility, we use and disclose your protected health information to treat you, to obtain payment for services and to conduct normal business known as health care operations. We may also share information with a contracted business associate who must meet our privacy requirements. Examples of how we use and disclose your information include:<\/p>\n\n\n\n<ul class=\"wp-block-list is-style-checkmark-list\">\n<li><strong>Treatment&nbsp;<\/strong>\u2013 We document each visit and\/or admission. This documentation may include your test results, diagnoses, and medications, and your response to medications or other therapies. This allows your doctors, nurses and other clinical staff to provide the best care to meet your needs.<\/li>\n\n\n\n<li><strong>Payment&nbsp;<\/strong>\u2013 We document the services and supplies you receive at each visit or admission so that you, your insurance company or another third party can pay us. We may tell your health plan about upcoming treatment or services that require its prior approval.<\/li>\n\n\n\n<li><strong>Health care operations&nbsp;<\/strong>\u2013 Medical information is used to improve the services we provide, to train staff and students, and for business management, performance improvement, and customer service.<\/li>\n<\/ul>\n\n\n\n<div class=\"wp-block-columns is-layout-flex wp-container-core-columns-is-layout-28f84493 wp-block-columns-is-layout-flex\">\n<div class=\"wp-block-column is-layout-flow wp-block-column-is-layout-flow\" style=\"flex-basis:45%\">\n<p>We may also use the information to:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Recommend treatment alternatives<\/li>\n\n\n\n<li>Tell you about health benefits and services<\/li>\n\n\n\n<li>Communicate with other UMass Memorial OHCA members or business associates for treatment, payment or health care operations<\/li>\n\n\n\n<li>Send appointment reminders<\/li>\n\n\n\n<li>Communicate with family or friends involved in your care with your permission<\/li>\n\n\n\n<li>Include you on the inpatient list for callers or visitors if you are admitted*<\/li>\n\n\n\n<li>Respond to media inquiries should they be made*<\/li>\n\n\n\n<li>Let clergy know if you are admitted*<\/li>\n\n\n\n<li>Contact you about support for the UMass Memorial Foundation (fundraising)*<em>Services followed by an asterick (*) are optional. Tell the admitting clerk or fundraiser (if contacted) that you do not wish to participate<\/em>.<\/li>\n<\/ul>\n<\/div>\n\n\n\n<div class=\"wp-block-column is-layout-flow wp-block-column-is-layout-flow\" style=\"flex-basis:55%\">\n<p>There are limited times when we are permitted or required to disclose medical information without your signed permission. These situations include the following:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>For public health activities such as tracking disease or medical devices<\/li>\n\n\n\n<li>To protect victims of abuse or neglect<\/li>\n\n\n\n<li>For federal and state health oversight activities such as fraud investigations<\/li>\n\n\n\n<li>For judicial or administrative proceedings<\/li>\n\n\n\n<li>If required by law or for law enforcement<\/li>\n\n\n\n<li>To coroners, medical examiners and funeral directors<\/li>\n\n\n\n<li>For organ donation<\/li>\n\n\n\n<li>To avert serious threat to public health or safety<\/li>\n\n\n\n<li>For specialized government functions such as national security and intelligence<\/li>\n\n\n\n<li>To workers\u2019 compensation if you are injured at work<\/li>\n\n\n\n<li>To a correctional institution if you are an inmate<\/li>\n\n\n\n<li>For research following strict review to ensure protection of information<\/li>\n<\/ul>\n<\/div>\n<\/div>\n\n\n\n<p>Other uses and disclosures not previously described may only be done with your signed authorization. You may revoke your authorization, in writing, at any time.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" style=\"padding-top:var(--wp--preset--spacing--20)\">Our responsibilities<\/h2>\n\n\n\n<p>UMass Memorial is required by law to maintain the privacy of your medical information, provide this notice of our duties and privacy practices, and abide by the terms of the notice currently in effect. We reserve the right to change privacy practices and make new practices effective for all the information we maintain.
