To ensure the best health for you and all Clark students, you will need to have your health care provider complete the required <a href=\"https:\/\/www.clarku.edu\/offices\/health-services\/health-forms\/\" target=\"_blank\" rel=\"noreferrer noopener\">graduate immunization forms.<\/a> Once you have the complete forms, upload them to your Clark student portal as immunization forms.<\/p>\n\n\n\n<p>Per Massachusetts state law, full-time international students must be enrolled in Clark\u2019s student <a href=\"https:\/\/www.clarku.edu\/offices\/student-accounts\/tuition-and-fees\/health-insurance\/\" target=\"_blank\" rel=\"noreferrer noopener\">health insurance plan<\/a>.
