The principal processes addressed in his work include intimacy, acceptance, and motivating the adoption of relationship healthy practices. The proposed continuation effort will expand on a recently completed randomized clinical trial evaluating the efficacy of a brief behavioral intervention called the Marriage Checkup in enhancing marital functioning in US Air Force couples. The proposed continuation will evaluate the efficacy of the Marriage Checkup in an active duty US Army population and include additional outcome measures not included in the previous clinical trial. The additional measures will focus specifically on military occupational functioning, e.g. job satisfaction, job performance, reenlistment intention.&lt;\\\/p&gt;\\n&lt;p&gt;Several studies have found that marital distress is significantly associated with poorer work performance. Studies suggest that men in distressed marriages will, on average, lose 38 more days of work per year than men in non-distressed marriages (Forthofer, Markman, Cox, Stanley, &amp; Kessler, 1996). In a 12-year longitudinal study, marital quality and job satisfaction were found to be bidirectionally related, but the effect of marital quality on job satisfaction was found to be the strongest direction of effect (Rogers &amp; May, 2004). The study also found that increases in marital satisfaction were significantly related to increases in job satisfaction, and decreases in marital satisfaction were significantly related to decreases in job satisfaction. In a population-based sample of 2,677 married or cohabiting individuals, relationship discord was found to be robustly associated with work role impairment, defined as days respondents (1) were totally unable to work or carry out their normal activities, (2) had to cut down on what they did or did not accomplish as much as usual, or (3) required an extreme effort to perform up to their usual level at work (Whisman &amp; Uebelacker, 2006). In addition to greater work role impairment, Whisman and Uebelacker found that relationship discord was associated with greater general distress, poorer perceived health, and greater likelihood of suicidal ideation. Furthermore, meta-analyses have found that work-family conflict is negatively related to various indicators of employee well-being, including job dissatisfaction, burnout, physical symptoms, and poorer job performance (Amstad, et al., 2011; Michel, et al., 2009; Gilboa, Shirom, Fried, &amp; Cooper, 2008).&lt;br&gt;Finally, in a recent uncontrolled study of the effect of couple therapy on work functioning, it was found that improvements in relationship health associated with participation in couple therapy were significantly associated with improvements in work functioning from pre- to post-therapy. Improvements found had a respectable effect size (d = 0.34 overall and 0.71 for couples in the clinical\\\/impaired range; (Zahl-Olsen, et al., 2019), providing the first evidence that efforts to improve relationship health has the potential to simultaneously improve work functioning. Thus, the current literature provides robust support for the hypothesis that interventions designed to prevent marital discord and improve overall marital health can lead to significant improvements in force readiness. However, there have been no well-controlled clinical trials to date directly addressing the potentially significant role that directly addressing relationship health could have on associated work role impairment.&lt;br&gt;The Marriage Checkup (Cordova, 2009; Cordova et al., 2014) is a brief relationship health intervention designed to be the relationship health equivalent of other existing health checkups such as annual physical and dental health examinations. In 2013, the investigators embarked on an effort to adapt Marriage Checkup for use by behavioral health consultants in Air Force primary care. An open trial of Marriage Checkup in primary care was completed with active duty couples and the results supported both feasibility and effectiveness (Cigrang et al., 2016; Cordova et al., 2017). The investigators recently completed an RCT of Marriage Checkup adapted for primary care with 244 enrolled military couples. Analyses of baseline data found that participating couples reflected a wide range in level of couple distress including a sizable proportion seeking preventive care. The couples were also demographically representative of the Air Force, e.g., 78% enlisted. Rate of follow-up survey completion was high at both 1 month (89%) and 6 months (86%). Recently completed analyses of primary outcomes found that Marriage Checkup significantly improved the relationship health of treatment couples compared to couples in a wait list condition and these gains were maintained at a 6-month follow-up period. Effect sizes were in the small to moderate range across couple and individual functioning outcomes. However, the RCT was not designed to test for the potential effects of Marriage Checkup on the work functioning of enrolled couples.&lt;br&gt;The Marriage Checkup requires further testing in another military branch (e.g., US Army), to ensure readiness for potential roll-out DoD-wide in the future. The US Army is an ideal population for inclusion in the continuation population because Army personnel have experienced significantly greater stressors (e.g., more deployments, injuries, killed in action, TBIs, PTSD symptoms, etc.) than the other branches of the US military. These factors add additional challenges to maintaining healthy marital relationship in active duty Army couples. The proposed continuation project will be conducted in collaboration with the South Texas Research Organizational Network Guiding Studies on Trauma and Resilience (STRONG STAR) Consortium. STRONG STAR has successfully conducted 16 RCTs of psychological health interventions with active duty Army personnel at Fort Hood. STRONG STAR has an already established extensive research infrastructure embedded into Fort Hood.&lt;br&gt;Research Strategy with Aims. An RCT of Marriage Checkup on force readiness and work functioning should ideally utilize a multi-method approach to measurement to assure the highest quality data. Firstly, we will capitalize on well-established measures of the interface between relationship functioning and work outcomes that are currently available in the industrial-organizational psychology literature (proximal measures). These include measures of perceptions of family-supportive work environment (Allen, 2001), work-spouse conflict (Day &amp; Chamberlain, 2006), family performance (Frone, Yardley, &amp; Markel, 1997) and job satisfaction (Bowling, &amp; Hammond, 2008). Secondly, we will examine available objective indicators of participant work functioning including deployment readiness, loss of work due to illness, and intention to reenlist (distal measures).&lt;br&gt;Aim 1: To conduct a randomized controlled trial to evaluate the effectiveness of the Marriage Checkup in Primary Care compared to usual care in improving couple\\u2019s relationship health in US Army couples.&lt;br&gt;Aim 2: To evaluate the effectiveness of Marriage Checkup compared to usual care in improving occupational functioning.&lt;br&gt;Research Design &amp; Methods. The randomized trial will recruit U.S Army couples from Joint Base San Antonio and Fort Hood, Texas in affiliation with the University of Texas Health Science Center at San Antonio's STRONG STAR (South Texas Research Organizational Network Guiding Studies on Trauma and Resilience) Consortium. Participants will be U.S. Army active duty or activated reserve members and their partners who are currently married. Study exclusion criteria will mirror the clinical guidelines for patients typically not seen by Behavioral Health Consultants working in primary care clinics, i.e., moderate or higher suicide risk, psychosis or unmanaged bipolar disorder, alcohol or substance dependence in the past 8 weeks. In addition, potential participants will be excluded if they are currently in other talk-therapy marriage counseling. Participants will be randomly assigned to either receive Marriage Checkup immediately or to a six month wait list condition. At the conclusion of six months, the wait list participants will be offered the opportunity to receive the Marriage Checkup. All participants will complete a baseline assessment prior to randomization and follow-up assessments immediately post-treatment and one-month and six months post-treatment.&lt;br&gt;Statistical Analysis Plan. To account for clustering of observations within people and within couples, data will be analyzed using a 3-level multilevel model with repeated measures (level-1) nested within individuals (level-2) nested within couples (level-3). Site level clustering will be included as a level-3 fixed effect. Employment status will be included as a level-2 predictor, with employment status by time and treatment interactions. The outcome of interest will be the treatment by time interaction term. Significant effects will indicate that the treatment group had a significantly different rate of change on force readiness and work functioning than the control group. The delta method will be used to calculate the total effect of treatment at the endpoints.&lt;br&gt;Power Analysis. We conducted a simulation study guided by results from our recent largescale trial. Relationship health has been shown to directly affect both mental health (e.g., depression, substance use, suicidality) and physical health (e.g., immune system functioning). The barriers to relationship help seeking are high, including the time, cost, stigma, and the challenge of admitting the need for relationship help. Although evidence suggests that couple counseling for already distressed couples is effective, the percentage of military personnel in distressed relationships who attend therapy is alarmingly low. Thus, there is a substantial need in the military for early detection and prevention of deteriorating relationships. The Relationship Checkup (RC) addresses this issue by providing a less-threatening option in primary care for couples to seek early preventive relationship help before they become distressed. Our previous DoD-funded Relationship Checkup RCT with US Air Force couples showed that the RC in primary care successfully attracted couples from across the relationship health spectrum, from non-distressed, moderately distressed, to severely distressed, including a substantial number of couples who would never have otherwise sought relationship assistance. Targeting upstream relationship health with the RC for primary care has the potential to decrease the risk for multiple adverse outcomes including depression, suicide risk, and substance abuse. Consistent with the sub-area 2d of the focus area Prevent and Assess, the proposed project will examine the RC for primary care\\u2019s effects on multiple indicators of the work-family interface and occupational functioning. Military stakeholders who work with service members and their families (e.g., military leadership, behavioral health providers) generally presume that relationship health contributes to occupational functioning. Unfortunately, no published study to date has tested the effect of DoD family-strengthening programs on the work functioning and operational readiness of active duty military. In addition, no study to date has examined whether the presence of traumatic brain injury (TBI) in one partner effects the outcomes of family-strengthening programs. This project was previously submitted to the FY 2021 Military Operational Medicine Research Program\\u2019s Open BAA and received an overall score of 2.4 (Good). Unfortunately, the scientific literature supporting this presumption is relatively small and based mostly on cross-sectional research designs (National Academies of Sciences, Engineering, and Medicine, 2019). No randomized controlled trial (RCT) has examined the effects of couple\\u2019s therapy on occupational functioning within a military or civilian sample. Such research is critically needed because it could lead to scalable interventions that improve the marital health and occupational functioning of military personnel, thus contributing to mission readiness. To address this gap, the proposed research would conduct an RCT to examine the effects of the Marriage Checkup (Cordova, et. al., 2014) on multiple indicators of occupational functioning within a sample of U.S. Army personnel. The proposed study is an extension of a recently completed RCT that demonstrated the efficacy of the Marriage Checkup in an active duty U.S. Air Force population (Principal Investigator (PI); Jeffrey Cigrang, W81XWH-15-2-0025). As a result of the successful completion and strong findings of this previous project, the PI was specifically asked at the 2019 Military Operational Medicine Research Program (MOMRP) In-Progress Review to develop and submit a follow-on proposal to examine the effects Marriage Checkup on occupational functioning in a different branch of the military (e.g., U.S. Army).
