Overview
Students with a documented disability requesting Alternate Format Textbooks must submit their request three weeks in advance of when the semester starts. This allows Disability Services to request and receive books in a timely manner. Disability Services will make effort to procure the requested item as soon as the request is received. Students are strongly encouraged to submit their requests sooner than later.
When a requested book is not available in electronic format, then the publisher must send a complimentary copy for scanning so that the student does not destroy his or her own purchased version. Scanning into electronic format entails removing the binding/covers, and scanning the pages into PDF. This may be done at the Clark U copy center. The student bears the cost of the printing if they want more than just the electronic file. Student may use his or her own copy immediately rather than waiting for the publisher's copy.
- Student must complete this request form three weeks before the book is needed (all fields are required).
- Failure to complete this form will result in either a delay or not receiving the book(s) in alternate form.
- Students must provide all information requested on this form. This form is the only way books requests will be accepted. E-mail requests will not be accepted.
- Please save your textbook purchase receipts and submit a copy to Disability Services for our records.
