Important Immunization Information
Massachusetts State Law requires all full-time students who enroll in a Massachusetts public or private college or university to be immunized against Diphtheria, Tetanus, Measles, Mumps, and Rubella, Hepatitis B and **Meningitis. In addition, students must complete a tuberculosis risk assessment and be tested for exposure to the disease, if required.
Students who have not fulfilled the state mandated health immunization requirements will not be permitted to register for classes. For new students, an immunization record must be fully completed, signed by a physician, high school nurse or other health provider, or a student can attach a school or military immunization record to Clark's immunization record. There are 3 forms attached to this mailing. They must be completed and returned to your department or program no later than August 1, 2008.
Students are cleared for registration only after a Health Services nurse has reviewed a student's records and determines them to be in compliance with Massachusetts state law.
Proof of receiving the following immunizations/tests is required:
- Td (tetanus and diphtheria booster within 10 years)
- Two (2) doses of live vaccine for measles, mumps and rubella (one after 12 months of age and one
at least one month after the first)
- Three (3) doses of Hepatitis B vaccine
- Tuberculosis risk assessment/PPD test if required
- One dose of Meningococcal (Meningitis) vaccine** (for serogroups A, C, Y and W135 within the past five years or a signed waiver two (2) weeks prior to arriving on campus
**Required for newly enrolled full-time residential students only
All students should make arrangements for any needed immunizations with medical practitioners in their home country or state to avoid charges for immunizations and registration holds after arriving at Clark.
