Important Immunization InformationMassachusetts State Law requires all full-time students who enroll in a Massachusetts public or private college or university to be immunized against Diphtheria, Tetanus, Measles, Mumps, and Rubella, Hepatitis B and **Meningitis. In addition, students must complete a tuberculosis risk assessment and be tested for exposure to the disease, if required.Students who have not fulfilled the state mandated health immunization requirements will not be permitted to register for classes. There are 3 forms attached to this mailing. They must be completed and returned to your department or program no later than August 1, 2008. Students are cleared for registration only after a Health Services nurse has reviewed a student's records and determines them to be in compliance with Massachusetts state law. Proof of receiving the following immunizations/tests is required:
**Required for newly enrolled full-time residential students only If you have any questions regarding these requirements please call me at 508-793-7676 or email drobertson@clarku.edu or call Health Services at 508-793-7467 . Sincerely, Denise B. Robertson Graduate School Coordinator
